Vaccinations

No appointment is ever necessary for vaccinations at any of our three locations. Many of us live busy lives, and we at Martins understand that, so we have provided the information below to help expedite the process of getting vaccinated this year. We have also included the Vaccine Information Statements, supplied by the CDC, for each of the vaccinations we provide.

Flu season is upon us once again . Please print and fill out the “Consent and Release Form” in advance to ensure the fastest service possible. Bring the completed form and your insurance card (if applicable) to any of our three locations today!
